Reminder: D-SNP Model of Care training due Dec. 31, 2020
Providers play an integral role in the care teams that support our dual-eligible special needs (D-SNP) members. That’s why the Centers for Medicare and Medicaid Services (CMS) requires us to make sure providers who treat our PriorityMedicare D-SNP™ patients are trained on our Model of Care.
Training must be completed by Dec. 31, 2020.
What is our D-SNP Model of Care?
Our D-SNP Model of Care is a quality improvement tool that ensures the unique needs of our D-SNP members are met and describes the processes and systems we use to coordinate their care.
Who needs to complete D-SNP Model of Care training?
All providers who see PriorityMedicare D-SNP members must complete the training. Because PriorityMedicare D-SNP uses our Medicare network, you should plan to complete this training if you participate with our Medicare network.
How do I complete the training?
Training can be completed using one of two options. If you’re not sure which option you should use to complete training, contact your provider performance specialist.
- Option 1: Access the training as an on-demand webinar. Training takes approximately 15 minutes to complete. You can self-attest to completion of the training within the online training tool.
- Option 2: Some practices and providers may receive Priority Health Model of Care training directly from their organization, as part of other annual training requirements. The organization is then responsible for sending attestation that their providers have completed the training.
Training must be completed and attested to by Dec. 31, 2020.
