We’re bringing automation to provider information submissions

On April 22, 2021, we’re moving to an automated process for credentialing, provider enrollment and provider information change forms. You can learn more about this change by attending the April 22 Virtual Office Advisory (VOA) visiting our credentialing page page or provider changes page.

What's changing?

You no longer need to worry about faxing or emailing our credentialing and enrollment forms. Instead, you'll be prompted through our automated online submission process when downloading one of our forms.

Our new submission process will:

  • Allow you to directly submit your changes online
  • Ensure your information is submitted on the correct form(s) by guiding you through a short list of questions
  • Deliver forms instantly to the correct processing team which reduces the administration time you spend ensuring your provider updates are submitted correctly
