Medication Therapy Management Program

You can refer your group and individual HMO, POS, PPO and Medicare Advantage patients to a specially trained Medication Therapy Management pharmacist for a free consultation (Medicaid/Healthy Michigan Plan members are not eligible). In addition, our MTM pharmacists directly contact members identified as taking multiple medications to manage chronic conditions, offering them free consultations. 

What a consultation includes

Comprehensive medication review

A review of a patient’s medications, including prescription, over-the-counter (OTC) medications, herbal therapies and dietary supplements, that is intended to aid in assessing medication therapy and optimizing patient outcomes. This review may identify therapeutic duplications, drug-drug interactions, and can help patients organize their medication schedules.

Non-prescription consult

Helps patients figure out which over-the-counter drugs to use to treat minor ailments easily and inexpensively.

Drug information

Explains the purpose and correct use of new medications, and follows up with the patient to make sure everything is right.

Prescriber communications

Patients leave their appointment with a report, which may be sent to the patient's primary care provider.
