Healthy behaviors ahead

Make a healthy change and keep your plan.

As a Healthy Michigan Plan member, you may know that you need to choose a healthy behavior each year, such as stopping smoking or getting more exercise, and let your primary care doctor know. However, this year, if you don’t choose a healthy behavior by April 1, 2018, you may lose your enrollment in your Healthy Michigan plan and be placed in a Marketplace plan.

Choosing a healthy behavior

There are a variety of healthy behaviors you can choose from, including getting preventive care during your annual checkup, completing your Health Risk Assessment and selecting to receive information on improving your health or managing chronic conditions, such as smoking or obesity.

What do you need to do?

See your primary care doctor and engage in a healthy behavior before the April 1, 2018 deadline. By choosing a healthy behavior, you can keep your plan and make changes to improve your health and your life.
