If you don't have premium assistance
If you're enrolled in a Priority Health Medicare plan (PriorityMedicare℠, PriorityMedicare Value℠, PriorityMedicare Ideal℠, PriorityMedicare Merit℠, or PriorityMedicare Select℠) then you may qualify to receive premium assistance on your Part D drug prescriptions.
How to find out if you qualify for getting extra help
If you know you qualify for premium assistance, there is no need to apply. You automatically qualify for premium assistance if you're eligible for:
- Supplemental Security Income (SSI)
- Medicare savings program
You may receive (or may have already received) a letter from Medicare or the Social Security Administration (SSA) about your premium assistance eligibility for this plan year.
If you have not received a letter from Medicare or the SSA, but think you may qualify for premium assistance, call:
- 1.800.MEDICARE. TTY/TDD users should call 1.877.486.2048, 24 hours a day/7 days a week.
- The Social Security Office at 1.800.772.1213 between 7 a.m. and 7 p.m., Monday through Friday. TTY users should call 1.800.325.0778.
