Priority Health Member Rewards Program cyber security incident

Priority Health discovered a cyber security incident on Dec. 16, 2021 involving its Member Rewards Program in which an unauthorized individual accessed several Priority Health Member Portal (PHMP) accounts.

After conducting a thorough investigation, the company has determined that the unauthorized individual was able to view information within PHMP accounts that may have included names, dates of birth, addresses, phone numbers, email addresses, insurance information, claims information and limited medical information.

There is no evidence that Social Security Numbers were accessed, and the investigation has not revealed any misuse. However, out of an abundance of caution, Priority Health has alerted all members and employers who were potentially impacted by this incident, via mailed letter, and is providing a variety of services and measures to help safeguard those individuals. What happened?

An unauthorized party gained access to Priority Health member accounts between Nov. 24, 2021 and Dec. 16, 2021. The information compromised may have included personal information. We do not know if any of the information was read. We apologize that this happened.

What information was involved?

The unauthorized party may have viewed information such as names, dates of birth, addresses, phone numbers, email addresses, insurance information, claims information, and limited medical information. At this time, we have no reason to believe Social Security Numbers were viewed.

What we are doing.

We took steps to increase our security. We often review our practices to improve the security and privacy of personal information, which is very important to Priority Health.

What impacted members can do.

We are offering free credit monitoring to impacted members for two years. The free services will be provided by Experian.

How do impacted members enroll for the free services?

Impacted members can enroll in credit monitoring services for free by visiting: They must enroll by: May 31, 2022. (Codes will not work after this date.)

When prompted, impacted members must provide the unique code in their letter to receive services. Please note that this code is case sensitive.

Additional steps to protect your credit.

Contact any one of the three major credit card agencies and have a fraud alert placed on your credit file. A fraud alert lets creditors know to contact you before new accounts are opened in your name. You will also be sent copies of your current credit files. You only need to call one of the credit agencies, but the fraud alert will be placed in all three files.

Examine your credit reports carefully. Look for:

  • Accounts you did not open
  • Requests from creditors you did not start
  • Personal information, such as your home address and Social Security number to make sure they are accurate

If you see something you do not understand, call the credit agency at the telephone number on the report.

Check your credit report every three months for the next year even if you do not find signs of fraud.
