Family combined deductible

Your health savings account (HSA) works as a point of service (POS) health plan, which means covered services are paid at either a Preferred level or Alternate level, depending on how you seek services. See your insurance policy for details.

Your Family Combined Deductible is the total cost of services (Preferred and Alternate level) for all individuals on your policy that you must pay before your plan begins to pay benefits.

• All services apply toward the combined deductible except for Preferred level preventive care (see Preventive Health Care Guidelines for details).
• This deductible must be met each plan year.
• Once you have met your Family Combined Deductible, your plan benefits will be applied.
