Find your plan

Look at your ID card to find which Priority Health plan you have.

If you're still unsure after finding your health plan, check the Group Name field in the middle of your card.

• If the Group Name is your employer, you have an Employer-sponsored plan.
• If there is no Group Name, you have an Individual plan.
