Medicaid/Healthy Michigan coverage level

The drug coverage level tells you whether or not a drug is covered:

• Covered: Priority Health Medicaid or Healthy Michigan Plan pays for this drug.
• Not covered: Drug that your prescription plan will not pay for or that is not included on the approved drug list. Call Customer Service for more info: 888.975.8102.
• Conditional: Priority Health Medicaid or Healthy Michigan Plan may pay for this drug if you meet the specific requirements listed under coverage details.
• Excluded: Drug that is not covered per your plan documents and is never covered for any reason.
