What are coverage documents?

Coverage documents explain how your health coverage works. They include:

• A Certificate of Coverage, Policy, Evidence of Coverage, or Summary Plan Description, depending on your plan
• Any riders or addendums (additions, changes or deletions) chosen by you or your employer
• The Summary of Copayments and Deductibles
