Health Risk Assessment (HRA)

The health risk assessment is a confidential 40-question form that lets us get to know you better. The information you provide allows our Care Management team to connect you to a Care Manager who will also work with your doctor to create a personalized care plan with you.

As a PriorityMedicare D-SNP member, you will need to complete a health risk assessment when:

  • You’re first enrolled in the plan (within 90 days of your plan effective date)
  • On an annual basis, if you’ve renewed your plan for the year
  • There is a change in your health or caregiver status, you will need to update it

How to fill out your health risk assessment

The health risk assessment form takes about 30 minutes to complete. Choose one of the following ways:

    Call Priority Health Customer Service at 833.939.0983 (TTY 711). Select option 2, followed by option 3. We're available from 8 a.m. to 8 p.m. seven days a week.

The importance of completing your health risk assessment*

After completion, your confidential results will be submitted to the Priority Health D-SNP Care Management team. They will review your individual care needs and connect you to a Care Manager. Your Care Manager will work with your doctor to create a personalized care plan, guide you through the healthcare system, and connect you with any resources needed to make sure your health goals are met.
