Guiding you through the health care system

Your Care Manager works closely with your primary care provider (PCP) to create a Care Plan that will manage your health and keep it on track. They can help coordinate your care, make sure you have necessary medical supplies and work with you towards achieving your health and wellness goals.

They’ll also be there for you after a recent hospital stay by assisting you in scheduling follow-up appointments, reviewing changes to your medications and more.

Connecting with your Care Manager

Regular communication with your Care Manager, such as reviewing you Care Plan, will happen over the phone.

We encourage you to sign up or log in to your Priority Health member account, where you’ll have the ins and outs of your plan at your fingertips. You also have access to the My Care Management tool, where you can send a secure email to your Care Manager and review your personalized care plan.

If you need support with technology use or navigation, let your Care Manager know.
