Using your vision, dental and hearing benefits

Vision, dental and hearing services included in your plan provide you with holistic coverage to keep you at your best.

View your full vision, dental and hearing coverage details in your Certificate of Coverage.

Vision

Routine vision benefits through EyeMed can help you care for your eye health and offset the cost of eyewear. Covered services include:

  • $0 copay for one routine exam each year (includes dilation and refraction)
  • $0 copay for one retinal imaging per year
  • $100 eyewear allowance per year

As a Priority Health EyeMed member, you have access to thousands of independent providers as well as national retailers such as LensCrafters, Pearle Vision and Target Optical.

Routine vision services must be provided by an EyeMed "Select" provider. Call 844.366.5127 (TTY 711), Monday through Friday from 8 a.m. to 8 p.m. Or search here.

When you go to an EyeMed "Select" network provider, they file your claim and you only have to pay the cost of any services or eyewear that exceeds your plan's benefit. Show your Priority Health member ID card to health care providers when using your in-network vision benefits.

To see your full EyeMed coverage, please reference the Vision section in your Certificate of Coverage.

  • If you see an out-of-network dentist, you may pay more if you go to a dentist who is not in the Delta Dental network. If the dentist charges more than Delta Dental allows, you will need to pay the difference.
  • You’ll usually pay less with an in-network dentist.
  • Before seeing an out-of-network dentist, ask if they have opted out of Medicare. Delta Dental can’t pay for services from dentists who have opted out.

Medication Therapy Management program

As a Priority Health Medicare member, you have access to our Medication Therapy Management services, or MTM for short. The MTM program connects you with a registered pharmacist for a yearly comprehensive medication review that can help reduce your costs and improve your health.

Your MTM pharmacist is your personal resource and advocate who will:

  • Help you understand your medications including what they do, when and how to take them, what to expect, and how to minimize side effects.
  • Identify and help avoid possible conflicts between medications and/or over-the-counter products.
  • Discuss money saving options and alternatives for filling your prescriptions.

The preferred way of disposing unused controlled medications is to bring them to community take-back sites near you. Before bringing your medications to take-back sites, use a marker to cover any personal information, such as your name or prescription number. The US Drug Enforcement Administration (DEA) allows unused prescription medications to be mailed back to pharmacies or other authorized sites using packages made available at pharmacies or other drug disposal sites. Contact your local pharmacy prior to mailing unused medications. Other medication collection sites in your area and information about medication disposal may be found at the websites below:

*The Galleri test is available by prescription only. Galleri does not detect a signal for all cancers and not all cancers can be detected in the blood. False positive and false negative results do occur. Galleri is a screening test and does not diagnose cancer. Diagnostic testing is needed to confirm cancer. The Galleri test identifies DNA in the bloodstream shed by cancer cells and does not predict future genetic risk for cancer. The Galleri test should be used in addition to healthcare provider-recommended screening tests. Eligibility rules apply.
