Your plan includes routine vision from EyeMedSM providers, hearing coverage from TruHearingTM providers and preventive and comprehensive dental services through Delta Dental®.

You will receive your annual Low-Income Subsidy (LIS) letter that explains your LIS level and copay amounts for your plan year. This is also known as Extra Help.
Medication Therapy Management
Keep up to date on the medications you need with our Medication Therapy Management (MTM) program. We work with specially trained area pharmacists to help you get the best results from your medications while keeping your out-of-pocket costs down. These pharmacists do much more than dispense drugs: they review your current medications and go over any questions you may have.

Meeting you where you are in your health journey is important to us that is why we are offering a Priority Flex card to all of our members.
Priority Health has a D-SNP (HMO) plan with a Medicare contract and a contract with the State Medicaid program. Enrollment in Priority Medicare D-SNP (HMO) depends on contract renewal.
