Our Medicare Advantage network includes 97% of primary care doctors in lower Michigan, and all major hospital systems, so you have plenty of in-network options. Plus, you don’t need a referral to see an in-network specialist.

Change your primary care physician

You can change your primary care physician (PCP) in one of three ways:

1. In your member account, Go to My Plan, then choose Find a Doctor. Search for the provider you want, then choose Select PCP and follow the steps.
2. Call Customer Service at the phone number on the back of your member ID card.
3. Mail us a PCP change form. Download and complete your form before you mail.
