No-cost in-home bonus checkups

As a member of a Priority Health Medicare Advantage plan, you can schedule an in-home health assessment once a year. Think of this as a "bonus" checkup, intended to answer your questions and update your regular doctor.

For this assessment, a health care provider comes to you – no driving, no waiting room – and you get a comprehensive checkup at no additional cost in the comfort of your own home, the home of a friend or family member, or a location that's convenient for you. We want you to feel comfortable! You may also ask a friend or family member to be with you during your assessment.

Your participation won't affect your health plan coverage or any care you're receiving from your doctor. All of the information gathered during your health assessment may be shared with you and is sent to your regular doctor, to help you manage your care together. These visits not only help prevent health issues, they've been crucial in detecting problems before they get worse.

What to expect

An in-home health assessment usually takes about 45 minutes. It gives you time to discuss your medical history in depth with a licensed physician from our partner, Signify Health (previously known as CenseoHealth), and get a no cost medical assessment. There is no need to change your clothes for your appointment, there are no blood tests and no invasive procedures.

During the visit, you'll be able to:

  • Ask questions about your health
  • Share concerns that may have come up since your last doctor visit
  • Review your vital signs and medications

The health care provider will:

  • Take your blood pressure
  • Listen to your heart
  • Ask about your medications
  • Ask about your health history and other health issues
  • Explain more about your health
  • Offer you ideas and advice

After your checkup, the health care provider will give you a personal checklist of things to discuss with your regular doctor at your next appointment. A summary of your checkup will be sent both to you and to your regular doctor to help you both manage your health.
