Adding a provider to your practice
Through prism, our provider portal, you can add one or more providers who are already credentialed with us to your practice by:
- Log into your prism account
- Navigate to Enrollments & Changes
- Select Change Individual Provider or Organization
- Follow the directions as indicated and use the Provider Change Form provided
