Starting Oct. 1, 2026, new individual providers enrolling with Priority Health for Medicaid only will use a new standardized credentialing and enrollment form. This change is in alignment with the Michigan Department of Health and Human Services (MDHHS) new requirements for Michigan’s Medicaid Health Plans (MHPs).
Process
New individual providers who are enrolled in CHAMPS and wish to participate in Medicaid only should:
- Complete the MAHP Provider Enrollment Form.
- Email the completed form to phleadershipprovidernetworkcredentialingandenrollment@PriorityHealth.com.
Within 10 business days of submission (with a valid, active contract), the Priority Health team will follow up with you to request additional information or confirm that a completed application was received.
If you don’t receive this confirmation email within 10 business or your request isn’t complete within 90 days, email exceedsprocessingtime@priorityhealth.com for assistance.
Not impacted by this change
- Organizational providers (facilities that meet specific criteria to be credentialed and screened as organizations)
- Credentialing applications for commercial or Medicare plans, which must be submitted via our existing process in our portal
- Providers already credentialed with us
- Providers who submitted a Medicaid credentialing application online before Oct. 1, 2026
