JA, JB: Route of administration modifiers

Modifier descriptions 

  • JA - ADMINISTERED INTRAVENOUSLY 

  • JB - ADMINISTERED SUBCUTANEOUSLY 

These modifiers are used when one HCPCS code for a drug has multiple routes of administration. 

Drugs given by intravenous infusion would be reported with the JA modifier. 

Drugs given by subcutaneous injection would be reported with the JB modifier. 

Denials will occur when the appropriate JA or JB modifier is not applied to the billed drug, which has more than one route of administration.
