Earn rewards for taking care of your health

Your Priority Health Medicare plan includes the Healthy Rewards program, designed to encourage and celebrate your commitment to wellness. When you complete eligible healthy activities like getting your flu shot or annual physical, you’ll earn rewards you can spend at popular retailers.

How it works

Participating is simple and rewarding:

  1. Complete healthy activities like screenings, exams or vaccinations.
  2. Record your activities in the Rewards Tracker at priorityhealth.com/rewards or by calling 833.415.4380.
  3. Earn rewards that are automatically loaded to a separate “purse” on your OTC card.
  4. Spend your rewards1 in store at the following participating retailers: 
  • Albertsons
  • Meijer
  • Costco
  • Target
  • CVS
  • Walgreens
  • Dollar General
  • Walmart
  • Kroger

How do I get a rewards card?

For Priority Health Medicare Advantage plan members with an over-the-counter (OTC) benefit, rewards will be loaded onto the OTC card. One card will be used for their OTC allowance and Healthy Rewards. If the member doesn’t have an OTC benefit, they’ll receive the rewards card after reporting their first healthy activity. It may take 7-10 business days to receive the card after reporting the first activity.

How can I check my rewards card balance? 

The rewards balance can be viewed in the Rewards Tracker at priorityhealth.com/rewards, or by calling Priority Health customer service at 833.415.4380 (TTY 711).

Get started today

Log in to your account at priorityhealth.com/rewards. If you already have an over-the-counter (OTC) account, you will use the same credentials to log in. If you need to set up an account, refer to the steps below.  

  1. Go to priorityhealth.com/rewards and click “Set up Access.”
  2. Enter your last name as it is listed on the back of the enclosed card (including any suffixes in your name, like Jr.), your date of birth and your Priority Health member ID (including the dash and two digits following the dash). 
  3. Accept the terms and conditions and the privacy policy by clicking in the boxes. Then click “Continue.”
  4. You will be taken to a new screen to enter your email address. 
  5. A code will be sent to your email address. Enter that code on the account setup screen and click “Submit.”
  6. Check your email once more for an auto-generated password to use the first time you sign in. Use that password to sign into your account. Once you are in your account you are able to change your password.
    Your account uses two-factor authentication for your safety. You will be asked to enter a code each time you log in. The code will be sent to your email address.

Program runs January through December each year. Rewards for services received in 2026 must be redeemed before December 31, 2026. 

1 Rewards cannot be used to purchase alcohol, firearms, lottery tickets or tobacco.
