The health risk assessment is a confidential 75-question form that lets us get to know you better. The information you provide allows our Care Management team, along with your doctor, to create a personalized case plan with you.
As a PriorityMedicare Dual Premier member, you will need to complete a health risk assessment when:
- You're first enrolled in the plan (within 90 days of your plan effective date)
- On an annual basis, if you've renewed your plan for the year
- There is a change in your health or caregiver status, you will need to update it
How to fill out your health risk assessment
Your care manager will be calling within the first 90 days to schedule a time to complete the assessment with them. You can also fill out the assessment:

Call Priority Health Customer Service at 833.939.0983 (TTY 711). Select option 2, followed by option 3. We're available from 8 a.m. to 8 p.m. seven days a week.
Included in your enrollment materials is a paper version of a pre assessment that allows your care manager to get to know you before scheduling the Health Risk Assessment appointment. You can return this by mail using the pre postage return envelope.
