If you have both Medicare and full Medicaid benefits, you might be eligible for a Highly Integrated Dual Eligible (HIDE) Special Needs Plan (SNP), like PriorityMedicare® Dual Premier.

What is a HIDE SNP and what does it include?

With a Priority Health Highly Integrated Dual Eligible Special Needs plan, your Medicare and Medicaid benefits are rolled into one simple health plan, with one member ID card for all benefits. You’ll still have all the coverage provided by both Medicare and Medicaid. Plus, as a Priority Health member, you can expect award winning1, Michigan-based customer service and a support team to help you get the care you need. And you will also receive extra benefits to help you stay healthy.

  • Routine dental through Delta Dental®
  • Routine vision coverage and routine hearing exams and hearing aid coverage
  • A monthly PriorityFlex allowance to use towards over-the-counter (OTC) items and if eligible, healthy food and produce, meal delivery, select utilities, pest control services, household supplies and personal care items.2
  • $0 tier 1 and tier 2 prescription coverage on our Approved Drug List
  • A Care Manager who is a health professional and will help you get care and services that affect your health and wellbeing
  • Fitness benefit through One Pass® with access to the largest network of fitness locations plus more than 20,000 digital classes and one at-home workout kit per plan year3
  • Long term support services (LTSS) for eligible members, which provide help for people who need assistance to do everyday tasks
  • Transportation to medical appointments and services

PriorityMedicare® Dual Premier (HMO D-SNP) is a health plan that contracts with both Medicare and Michigan Medicaid to provide benefits of both programs to enrollees. Enrollment depends on contract renewal.
