What is a D-SNP and what does it include?

With a Priority Health D-SNP, your Medicare and Medicaid benefits are rolled into one simple health plan. You’ll still have all the coverage provided by both Medicare and Medicaid. Plus, as a Priority Health member, you can expect outstanding Michigan-based customer service and support from a care management team to help you get the care you need.

And you will also receive extra benefits to help you stay healthy.
