What is a care manager?

Care managers help you manage chronic conditions and coordinate your care so that you can focus on what’s important to you.

You may benefit from care management if you:

  • Were recently diagnosed or are currently living with a chronic condition
  • Have had recent hospital stays or emergency room visits
  • See multiple doctors or take several medications
  • Feel overwhelmed about managing your health
  • Need help with long-term planning, advance care planning or medical decisions

How to connect with a care manager

Care managers may be available to you at no additional cost depending on your plan type.

The first step is to reach out—connect with a member of our care management team in one of the following ways:

Call the phone number on the back of your member ID card.

Go to the My Care Management tool in your online Priority Health member portal.

The impact of care management
