Annual Wellness Visit
Your plan includes a $0 Annual Wellness Visit each calendar year. This is a chance to meet with your physician to develop or update a personalized prevention plan based on your current health and risk factors.
Caregiver support
Your plan comes with unlimited access to Carallel Care Advocates that provide resources and support on topics like social and emotional needs, financial and legal concerns, housing, transportation and family communication.

Care management
Every Priority Health D-SNP member is assigned a care manager at no additional cost. Your care manager works closely with your primary care provider (PCP) to create a care plan that will help you manage your health.
They can help coordinate your care, make sure you have necessary medical supplies and work with you towards achieving your health and wellness goals. They’ll also support you after a hospital stay by assisting you in scheduling follow-up appointments, reviewing changes to your medications and more.
Regular communication with your care manager will happen over the phone.
Getting started
To get started, simply complete a health risk assessment (HRA), which takes about 30 minutes.
You can complete the survey:

- By mail Mail in the paper version you received with your enrollment materials using the pre-paid postage return envelope

One Pass can help you reach your fitness goals, while finding new passions along the way. Find a routine that's right for you whether you work out at home or at the gym.*
One Pass includes:
- Access to the largest nationwide network of gyms, fitness locations and boutique studios
- Live digital fitness classes and on-demand workouts for all fitness levels and abilities
- Groups, clubs and social events near you so that you can meet like-minded people
Getting Started
The day your coverage begins, you can set up your account online, get your One Pass member code and get started right away.
- Create your account
- Access your One Pass member code, search the network of gyms to find locations in your area, access digital classes, your CogniFit subscription and other member benefits.
- (Optional) Download the One Pass app and log in to access your account, programs and content from your mobile device.
- If you already belong to an in-network gym: After your coverage begins, stop by the front desk of your gym with your One Pass member code to make sure you are set up and ready to go.
If you plan to join a participating gym or fitness center: Bring your One Pass member code with you and a staff member will help you set up your membership. If you forget your code, in-network gyms will be able to look it up for you.
PriorityFlex
With your plan, you receive a $70 monthly over-the-counter (OTC) allowance to use toward health and wellness products, and if you qualify,** toward:
- Healthy food and produce
- Pest control services
- Personal care items
- Household supplies
- Meal delivery
- Utilities like gas, water, sewer, septic, electric, internet and phone
To use your PriorityFlex allowance on more than OTC health and wellness items, you must meet the Special Supplemental Benefit for Chronically Ill (SSBCI) requirements. To see if you qualify, complete the form at priorityhealth.com/qualify.
Purchasing OTC items and home bathroom safety devices and modifications
Get started with your online member account at priorityhealth.com/shopOTC.
In your online member account, you'll be able to check your available balance, shop online, find participating stores, search eligible products, view your transaction history and more. The video below will show you how to set up and navigate your member account.
There are three ways you can shop for OTC items with your PriorityFlex benefit:
- Order online: View and purchase products online anytime at priorityhealth.com/shopOTC or through the OTC mobile app. To download the app, search “Priority Health OTC” in the Apple App Store or Google Play.
- Shop in-store: Shop at Costco (PIN required at checkout), Meijer, Kroger, CVS, Walgreens, Walmart, Target and other participating store locations near you. You can search for retailers near you at any time by going to priorityhealth.com/shopOTC and using the "Find a Store" tool.
A PIN will be required to shop at Costco. No PIN is required when shopping at other retailers.
To set your pin:
1. Log into your member account and click "Manage card" on your account dashboard.
2. Select "Card management" on the left hand navigation, then click on "manage"
3. A pop-up window will appear. When prompted, select "set/reset your PIN"
- Call for delivery: Call 833.415.4380 (TTY 711) to place an order after reviewing eligible OTC items. Have your selected product names, OTC card number and shipping information available.
Purchasing Healthy Food and Produce
Shop in-store: Shop at Costco (PIN required at checkout), Meijer, Kroger, CVS, Walgreens, Walmart, Target and other participating store locations near you. You can search for retailers near you at any time by going to priorityhealth.com/shopOTC and using the "Find a Store" tool.
Purchasing Utilities, Pest Control Services or Meal Delivery
Pay for pest control services and select utilities with your PriorityFlex card as you would with any other Mastercard. You can place meal delivery orders through your OTC account at priorityhealth.com/shopOTC.

SafeRide®
SafeRide provides no-cost transportation to and from medical appointments and other health-related locations. Get up to 30 one-way trips per year, within 100 miles with just a phone call. You may also submit for mileage reimbursement, up to 100 miles per one-way trip. Schedule a ride by calling 855.932.5418 (TTY 711) 6 a.m. to 6 p.m., Monday - Saturday. Rides must be scheduled at least 48 hours before your requested pick- up time.
