In-Home Health Assessment: How You Can Voice Your Health Concerns and Get the Care You Deserve

Care in your home, personalized to your needs? It’s not too good to be true—it’s a real benefit available to eligible Priority Health Medicare, Medicaid and Individual plan members.

If you get a call about a no-cost, in-home health assessment, it’s a benefit that’s part of your Priority Health coverage. Priority Health partners with vendors that offer in-home health assessments, which are medical visits you can use in addition to your regular primary care visits—all in the comfort of your own home. During a visit, a nurse practitioner or other health care provider will sit down with you to review medical history, medications and answer any questions about your health. Medicare members are even eligible to earn a $25 reward for completing an in-home health assessment. What is an in-home health assessment? 

An in-home health assessment is an opportunity for a face-to-face visit with a nurse practitioner or other health care provider in a comfortable setting of your choice. It could take place in your home or another preferred location. These assessments could help identify any ongoing health issues before they become serious. They are also a great opportunity to clear up any concerns or ask any questions you have about your health since your last medical visit. 

How do I schedule my in-home health assessment and what can I expect from my visit? 

Scheduling is simple – if you are an eligible Priority Health Medicare, Medicaid or Individual plan member, a vendor that offers in-home health assessments will contact you. Visits can be scheduled at your convenience.  

During your assessment, the health care provider will: 

  • Review your health history and medications
  • Discuss any over-the-counter vitamins or supplements you take
  • Conduct a brief physical examination, depending on your health status and concerns 
  • Talk through any preventive screenings you may need

How long will the visit take? 

Most visits take about 45 minutes. 

Does it have to be at my home? 

No, the visit can take place at a friend’s home or in another quiet setting appropriate for medical conversations. 

Will my primary care physician know about this visit? 

Yes, the provider at your in-home health assessment will send a summary of your visit to your regular health care provider, which you can review together during your next appointment. You can also ask for your own copy of the summary letter.
