Priority Health partners with vendors that offer in-home health assessments, which are medical visits you can use in addition to your regular primary care visits—all in the comfort of your own home. During a visit, a nurse practitioner or other health care provider will sit down with you to review medical history, medications and answer any questions about your health. How do I schedule my in-home health assessment and what can I expect from my visit?
Scheduling is simple – if you are an eligible Priority Health Medicare, Medicaid or Individual plan member, a vendor that offers in-home health assessments will contact you. Visits can be scheduled at your convenience.
During your assessment, the health care provider will:
- Review your health history and medications
- Discuss any over-the-counter vitamins or supplements you take
- Conduct a brief physical examination, depending on your health status and concerns
- Talk through any preventive screenings you may need
