Member changes their PCP

When a member seeks care from you but isn't assigned to you as their PCP yet, you may submit a PCP change form on the member's behalf once the member completes the form. This is especially common and important for Medicaid and Healthy Michigan Plan members.

Print/fax process

  1. The member completes the Primary Care Provider Change Form.
  2. You (or the member) fax the completed form to us.

Online process

The member logs in to their Priority Health online account and clicks the "Change doctor" option.

Phone process

The member calls Customer Service at the number on the back of their member ID card.

Effective date of changes

A PCP change becomes effective on the first of the month following the date we get the change form/request.
