2026 HIDE-SNP and D-SNP Model of Care training is now available and required for Medicare and Medicaid providers

Providers play an integral role in the care teams that support our special needs plans (SNPs) members. That's why the Centers for Medicare and Medicaid Services (CMS) requires us to make sure providers who are contracted with us to see Medicare or Medicaid members are trained on our Model of Care (MOC) every year. Our 2026 HIDE-SNP & D-SNP MOC training is now available. 

What’s new this year? 

  • This year, because we have added the PriorityMedicare Dual Premier (HIDE-SNP) plan to our existing PriorityMedicare D-SNP plan, the training is now required for providers in our Medicaid network, in addition to our Medicare network
  • There is one combined training for both of our SNPs, so completing this training once in 2026 fulfills the CMS requirement for both plans. 

Who needs to complete this training? 

  • All providers in our Medicare or Medicaid networks (even if you’re only contracted with one or the other)
  • Out-of-network providers who see at least five HIDE-SNP or D-SNP members
  • This includes specialists, ancillary providers and anyone part of an ICT (interdisciplinary care team) for a HIDE-SNP or D-SNP member. 

What’s included in the training? 

Our MOC is a quality improvement tool that ensures the unique needs of our SNPs members are met and describes the processes and systems we use to coordinate their care. This is a CMS requirement. The training takes about 15 minutes.  

How do I complete my training? 

We have two options for you to complete your training and then attest your completion with us.  

Option #1: Virtual training (the best option for individual providers) 

Training is available as an on-demand webinar if you want to complete this training individually. Provider registration for the on-demand webinar counts as attestation, which means no additional documentation is required. 

Option #2: Bulk attestations (the best option for ACNs) 

You can group our HIDE-SNP & D-SNP MOC training with other required training (like compliance training) so you can submit attestation for multiple providers at the same time. If you choose this option, you’ll need to:   

  1. Distribute training to your providers using this link. 
  2. To attest to training, fill out the roster template with providers who’ve completed training, carefully following the instructions at the top of the spreadsheet. Only the Priority Health MOC roster Excel sheet provided will be accepted to report your completion. 
  3. Send attestation rosters to DSNPtraining@priorityhealth.com. 

     When you submit an attestation roster, you’ll receive one of two automated messages:     

  • A confirmation email stating the roster was successfully processed.
  • An email stating the roster wasn’t processed and the reason(s) why.

For both options #1 and #2, be sure to submit the correct provider NPI.    

Please ensure you include the correct provider NPI number when submitting the provider roster or registering for online training. If the NPI is incorrect, the provider’s status will be marked "incomplete" in our system. To correct an "incomplete" status due to an incorrect NPI, resubmit the provider roster or re-register for the online training with the correct NPI.   

What’s the deadline to complete the training? 

Training must be completed and attested to by Dec. 31, 2026. Late submissions will not be accepted.      

What’s HIDE-SNP? 

The Highly Integrated Dual-Eligible Special Needs Plan, also known as HIDE-SNP, is a new type of dual-eligible plan replacing Michigan's coordination-only D-SNP plan type in 2026 and 2027. It combines Medicare, Medicaid and Long-Term Services & Supports (LTSS) into one plan for an integrated member experience. PriorityMedicare Dual Premier is Priority Health's HIDE-SNP now available to dual-eligible members in some southwest and southeast Michigan counties. Please refer to either the D-SNP or Dual Premier pages of our Provider Manual for further details.
