PriorityMedicare D-SNP® (HMO) Model of Care training for providers
Providers play an integral role in the care team that supports our dual-eligible special needs members. The Centers for Medicare and Medicaid Services (CMS) requires all special needs plans, like PriorityMedicare D-SNP® (HMO), to provide initial and annual Model of Care (MOC) training to providers who routinely see D-SNP members. Training must be completed on an annual basis.
What is a Model of Care (MOC)?
A MOC is a quality improvement tool and integral component for ensuring that the unique needs of each beneficiary enrolled in a special needs plan (SNP) are identified and addressed. The MOC is reviewed and approved by CMS. It describes the processes and systems used by a SNP to coordinate care for its members. It must include:
- A description of the SNP population
- Elements of care coordination
- An overview of care providers
- Quality measurement and performance goals
Who needs Model of Care training?
CMS requires MOC training if you’re a provider who sees PriorityMedicare D-SNP members. PriorityMedicare D-SNP shares the same network as our other Medicare Advantage plans, so if you’re a Priority Health Medicare provider, you should complete this training. Training needs to be completed by Dec. 31 of each year.
How can I access Model of Care training?
Our D-SNP MOC training is available as an on-demand webinar and takes approximately 15 minutes to complete.
If you’re part of a practice or provider group that is part of an organization that completes our D-SNP training at the same time as other annual training requirements, you’ll complete the training when it’s provided to you by your organization.
