Announcing our 2026 Provider Mini‑Grant recipients

We’re pleased to announce the recipients of the 2026 Priority Health Provider Mini-Grants Program. The providers selected represent a diverse group of practices committed to improving quality performance, enhancing patient care and addressing key care barriers across our communities. 

  • Genesys Physician Hospital Organization
    • Genesys will introduce retinal screening technology into select primary care practices to improve access to diabetic eye exams for patients with diabetes, reducing care barriers like transportation, scheduling delays and access to specialists.
  • Lakeshore Allergy PC
    • Lakeshore Allergy will incorporate point-of-care fractional exhaled nitric oxide (FeNO) testing within their outpatient allergy/immunology practice to improve asthma diagnosis and management, reduce reliance on subjective symptom reporting and help address disparities in asthma outcomes among underserved populations. 
  • Livingston Physician Organization
    • Livingston Physician Organization will use PSV (Primary Source Verifier)—a HIPAA-compliant, AI-powered tool—to automate quality measure identification and verification by extracting data directly from medical records and generating submission-ready MiHIN APS files, supporting improved accuracy and performance in HEDIS Quality measures. 

Each funded project supports our providers in their continuous efforts to close care gaps, improve outcomes and strengthen care delivery for Priority Health members. We look forward to partnering with them in these projects and sharing outcomes and best practices across our network.
