Important note: If you’ve already submitted an application but didn’t receive a confirmation of receipt, please resubmit. An issue was identified with the email address included in the original communication. This has been corrected to ensure delivery.
Program FAQs
Q: What’s the goal of this program?
A: The Provider Mini‑Grants Program supports provider‑led projects that improve quality performance and enhance patient care. Priority consideration will be given to initiatives focused on:
- Medicare Part D Star measures
- Osteoporosis Management in Women Who Had a Fracture (OMW) HEDIS measure
- Pediatric and adult immunizations
- Addressing care disparities
Q: Who is eligible to apply?
A: Applications are welcome from ACNs, provider groups and individual practices that:
- Participate in the Priority Health Provider Network
- Are located and practicing in Michigan
- Can complete the proposed project by Dec. 31, 2026
There is no limit to the number of applications that may be submitted by an ACN or provider. Projects will be evaluated based on proposal details and their potential impact on Priority Health members.
Q: When will funding be awarded?
A: Funding will be distributed to selected recipients via check in the spring, once the review and selection process is complete.
Q: How will projects be reviewed?
A: Applications will be reviewed by a panel of Priority Health stakeholders and selected based on alignment with program goals and quality improvement priorities.
Q: How do I submit an application?
A: Download and complete the Mini‑Grant Application Form and submit it to: providerminigrants@priorityhealth.com. Additional supporting materials may be included, if applicable.
