Patients enrolled in Priority Health’s Highly Integrated Dual Eligible Special Needs Plan (HIDE-SNP) plan may qualify for Long-Term Services and Supports (LTSS) through Carallel to help them safely receive care at home or in community or facility settings.
What’s LTSS?
LTSS supports independence and an improved quality of life for patients who need help with daily activities due to age, disability or chronic conditions. Covered LTSS services may include:
- Assistance with daily activities like bathing or dressing
- Support with household tasks and transportation
- Nursing facility care, when appropriate
Eligibility criteria
HIDE-SNP members may be eligible for LTSS if they require ongoing assistance with daily activities, which is determined by a level-of-care assessment.
What’s the referral process?
To refer a HIDE-SNP member for LTSS, contact their Care Manager directly or call our PriorityMedicare Dual Premier Customer Service line at 833.939.0983. Hours include:
- From Apr. 1 through Sept. 30, we’re available Mon.– Fri. from 8 a.m.–8 p.m. and Sat. 8 a.m.–noon.
- From Oct. 1 through Mar. 31, we’re available seven days a week from 8 a.m.– 8 p.m. ET.
