A look at our 2027 Disease Burden Accuracy Program

Published on Oct 26, 2026

We’ve used data from our 2026 Disease Burden Accuracy (DBA) Program, along with feedback collected from our providers and ACN partners, to build our 2027 DBA Program. (This program was formerly called Disease Burden Management, or DBM.)

Here’s a first look at what’s coming in 2027.

We’re making some changes to the program, aiming to award PCP efforts while still focusing on compliant disease burden capture.

Our DBA program focuses on partnering with providers to ensure members' chronic conditions are accurately identified, documented and coded based on the care delivered. This helps create a more complete picture of each member's health status, supporting better care coordination, improved health outcomes and appropriate resources to meet member needs.

Through the 2027 program, we aim to better align with provider workflows, strengthen engagement at the PCP level and support more complete identification, documentation and management of chronic conditions.

In 2027, the program will focus on one incentive – Chronic Condition (HCC) Recapture.

We’ve designed the 2027 DBA program around the Chronic Condition HCC Recapture incentive. Incentive   calculations will align with and support provider documentation and coding activities, with increased focus on documentation accuracy. We’ve consolidated the PCP visit and coding accuracy incentives within the HCC recapture incentive.

The program features enhanced opportunities to earn incentives through timely HCC recapture and strong overall group performance.

These opportunities in 2027 include:

  • Increased recognition of early-year chronic condition recapture activity
  • Additional performance-based rewards for groups demonstrating strong overall recapture results
  • More timely incentive distributions throughout the year, with final settlement following the close of the performance year

We’ll  calculate earnings at the practice level and send payments to the participating ACN,.
