As a reminder, the following 2026 PCP Incentive Program (PIP) deadlines are approaching. More information on these deadlines is available in our 2026 PIP Manual (login required).
Oct. 15: PCMH designation attestation in PRA
PCPs must be practicing at a Patient Centered Medical Home (PCMH) designated practice in the program year to be eligible for the Care Management and Behavioral Health Collaborative Care incentives. ACNs should report this designation to us in the PRA tool by the November attestation period, which takes place in October. PCPs not reported as part of a PCMH-designated practice in PRA by the November attestation period will be removed from the applicable measures (both numerator and denominator) at year-end settlement.

Nov. 1: Supplemental data due for Jan. 1 – June 30
The first supplemental data submission deadline for the 2026 PIP program is coming up on Nov. 1, 2026, for dates of service between Jan. 1 – June 30. ACNs should make sure to submit all* supplemental data for applicable dates of service by this deadline. This includes data submitted:
- Digitally through MiHIN, APS, HL7 and EPP
- Through medical records via fax, mail or SharePoint

ACNs: if you have a secure SharePoint folder set up with us but are experiencing access issues for any of your staff, email us at HEDIS@priorityhealth.com with your ACN site name along with a list of email address for everyone who needs access to the folder.
* Exception: you may still submit blood pressure and HbA1c results at any time of year, regardless of date of service.
Nov. 16: PCP-to-practice “subgroup” organization in PRA
ACNs must organize PCPs by practice using our PRA tool’s “subgroup” feature by the December attestation period, which takes place in November. Any PCP not assigned to a “subgroup” at the close of this attestation cycle on Nov. 16 won’t be included in 2026 PIP settlement calculations.
