As a reminder, the Transformation of Care measures in our 2026 PCP Incentive Program (PIP) have specific eligibility requirements with deadlines approaching.
Care Management questionnaire
Care management questionnaires are due by Sept. 1, 2026. Each ACN must submit at least one questionnaire to be eligible for the Care Management incentive. Contact your Priority Health Provider Network Management specialist for a link to the questionnaire.
Data Quality Collaborative meetings
To be eligible for this incentive, ACNs must participate in 2 of 4 offered meetings with Priority Health data analysts. There are two remaining meetings for the 2026 program year, to be held on Aug. 26 and Nov. 4. Contact your Priority Health Provider Network Management specialist for support.
PCMH designation attestation
PCPs must be practicing at a Patient Centered Medical Home (PCMH) designated practice in the program year to be eligible for the Care Management and Behavioral Health Collaborative Care incentives. ACNs should report this designation to us in the PRA tool by the November attestation period, which takes place in October. We honor the following designation programs: BCBSM, NCQA, URAC and Joint Commission. PCPs not reported as part of a PCMH-designated practice in PRA by the November attestation period will be removed from the applicable measures (both numerator and denominator) at year-end settlement.
Note: If you received an alert from PRA that PCMH attestation was due this month, you can ignore it. Attestation is due in October as outlined above.
