We offer a nutrition based In Lieu of Services (ILOS) program for eligible Priority Health Medicaid members. This is an optional benefit that provides medically tailored or healthy meals delivered to your home at no cost. Our goal is to help ensure our members have greater access to nutritious food to support with improving their healthcare goals.
- Medically tailored meals: Fresh or frozen home delivered meals that are ready to eat and medically tailored for a specific disease or condition.
- Healthy meals: Nutritionally balanced, home delivered meals consisting of a hot, cold, frozen or shelf-stable meal.
Who is eligible?
To be eligible for services, you must meet at least (1) of the following clinical risk factors and both social risk factors outlined below:
Clinical risk factors
- Have a nutrition-sensitive condition, including diabetes, congestive heart failure, chronic obstructive pulmonary disease (COPD), hypertension, human immunodeficiency virus (HIV), cancer with malnutrition, malnutrition, sickle cell disease, or renal disease.
- Be recently discharged from a hospital or skilled nursing facility within the last 60 days.
- Pregnant and currently have, had a history of, or is at risk for at least one (1) of the following:
- High-risk pregnancy,
- Pregnancy, delivery, or birth complication including gestational diabetes, preeclampsia, newborn low birth weight, hyperemesis gravidarum and other causes of dehydration, maternal low birth weight of less than 2500 grams, multiple pregnancy, or malnutrition.
- A former foster care youth in Foster Care Transitional Medicaid and has a nutrition-sensitive condition (see above).
- A child with elevated blood lead levels or childhood obesity.
- A child eligible for the CSHCS program.
Social risk factors
- Be at risk of nutritional deficiency or nutritional imbalance due to food insecurity, defined as being unable to obtain nutritionally adequate, medically appropriate, and/or safe foods.
- Live in the Priority Health Medicaid service area which includes West Michigan, Northern Michigan and the metro Detroit area (qualifying counties are indicated in green on this map).

If you are not happy with the answers that our representative has provided, you or someone acting on your behalf can formally request that Priority Health change the response or decision provided.
