A routine eye exam can spot early signs of health conditions such as diabetes and high blood pressure1. We're here to provide access to quality care and share information about changes to your plan.
What's changing?
Starting December 1, 2022, your routine vision care will now be managed through our partnership with EyeMed.
What can I do?
If you are happy with your current provider, please check to see if they are in EyeMed's network. If your current provider is not, or you would like to see a different participating provider, you can choose a new one in EyeMed's network:
- Visit the Priority Health EyeMed directory
- Call EyeMed Customer Service at 866.608.9378 (TTY 711).
Be sure to bring your member ID card to your appointment to confirm your coverage.
If you choose to see a provider who is not part of EyeMed's network, any care you receive from this provider will not be covered starting on December 1, 2022. You will have to pay the cost of any care or eyewear you receive.
