Your Priority Health Choice, Inc. Medicaid plan includes vision coverage through our partnership with EyeMed. To make sure your eyes are healthy and help you see the best you can, we cover the following services:

  • One eye exam every 24 months
  • One pair of glasses every 24 months
  • Eye glass frames
  • Contact lenses

Finding in-network providers

If you are happy with your current provider, please check to see if they are in EyeMed's network. If your current provider is not, or you would like to see a different participating provider, you can choose a new one in EyeMed's network:

You do not need a referral to get eye care. Be sure to bring your member ID card to your appointment to confirm your coverage.

If you choose to see a provider who is not part of EyeMed's network, any care you receive from this provider will not be covered. You will have to pay the cost of any care or eyewear you receive.
