If Priority Health requests records that contain hand-written signatures, we strongly recommend that you submit a signature log when you send us the medical records.
You should keep a signature log for each provider in the office. It should include:
- Printed name
- All versions of the signed name, in the provider's own handwriting
- All versions of initials, in the provider's own handwriting
If a provider has various signatures, all versions of their signature should be included in the signature log.
The signature log should not be completed by anyone other than the provider.
