Here are a few tips from the Priority Health Medicaid team to help keep you healthy this winter.

Don't lose your coverage

You must fill out a Medicaid renewal form each year to keep your Medicaid health coverage. If you do not complete your annual renewal, you may lose health coverage.

Make sure your contact information is up to date in your MI Bridges account so Priority Health and the Michigan Department of Health and Human Services (MDHHS) can mail your renewal form. If you have already received a form in the mail, be sure to fill it out and return it right away.

Ask your doctor about chronic kidney disease

Chronic kidney disease (CKD) is common in people with diabetes and hypertension. CKD often develops slowly and with few symptoms.

Many people don't realize they have it until it's advanced and requires extensive medical treatment. If you have diabetes or hypertension, ask your doctor if you're at risk of developing CKD. You can also take a quiz from the National Kidney Foundation to see if you're at risk.

Have diabetes? Schedule important health checks

When blood sugar levels aren't controlled over time, it can impact your heart, kidneys, eyes and more. It's important to schedule the following screenings throughout the year to help stay healthy: Blood pressure check, A1C blood test, retinal eye exam, and kidney evaluation.

If you don't currently have a provider, use our Find-a-Doctor tool below to find one in your plan's network.

Protect your vision

Eye exams are an important part of maintaining eye health, especially if you're diabetic. A retinal exam takes a picture of the back of your eye and shows the doctor if your diabetes is negatively affecting your vision. This test is painless and can help prevent future vision loss. Schedule your eye exam today, and don't forget to mention if you are diabetic and need a retinal eye exam.

Vision coverage is offered through our partnership with EyeMed. Click the link below to view the provider directory. If you have questions about your vision coverage, call Priority Health Customer Service at 888.975.8102.

How do you rate your health plan?

Your experience with Priority Health is important to us. During Spring 2024, Priority Health may send you a Consumer Assessment of Healthcare Providers and Systems (CAHPS) survey through our trusted vendor, Press Ganey. The survey asks questions about our services and your experience with us as a Medicaid member.

Completing the survey helps to make sure your health coverage and care meets specific quality standards.
