Narrow networks

We offer five narrow networks to our individual MyPrioritySM plan offerings, for a total of five. Our narrow networks are listed below.

We ask the following of members of narrow network plans and of providers:

Member responsibility: Members receive reduced cost-sharing in-network. Going outside of the network results in member liability for all costs. Care they receive outside their network will not be covered. 

Provider responsibility: Assist patients by referring them to providers within their network. Use Find a Doctor to see which providers are in your patient's network. Request authorization for transplants and in instances where services are not available within their network.

Narrow network: Spectrum Health Partners (SHP)

Available to: Individuals under 65 who are members of individual MyPrioritySM HMO narrow network plans

Identification: ID cards indicate "Spectrum Health Partners."

Counties: Kent only

In-network providers:

  • Hospitals: All Spectrum Health hospitals, including those outside Kent county
  • Physicians: Spectrum Health Medical Group, and physicians who denote a Spectrum Health hospital in Grand Rapids as their primary hospital affiliation
  • Ancillary facilities: All those available in the SHP network; examples include Mary Free Bed, Pine Rest and Forest View

Narrow network: MyPriority Beaumont Health Network

Available to: Individuals under 65 who are members of individual MyPriority HMO narrow network plans

Identification: Member ID cards indicate "Beaumont Health Network."

Counties: Macomb, Oakland and Wayne counties

Network providers:

  • Beaumont Health hospitals – Dearborn, Farmington Hills, Grosse Pointe, Royal Oak, Taylor, Trenton, Troy and Wayne 
  • All physicians employed by Beaumont Health
  • Any individual community physician with admitting privileges at Beaumont Health who is listed in our Find a Doctor directory as in-network in the Priority Health Beaumont Network

Narrow network: MyPriority Bronson Healthcare Partners (BHP)

Available to: Individuals under 65 who are members of individual MyPriority HMO narrow network plans

Identification: Member ID cards indicate "Bronson Healthcare Partners."

Counties: Kalamazoo, VanBuren, and portions of Calhoun

In-network providers:

  • Hospitals: Bronson hospitals (all campuses) 
  • Physicians: Bronson Medical Group physicians, Bronson Healthcare hospital-based physicians, and physicians who denote a Bronson hospital as their primary hospital affiliation through credentialing, on their CAQH application
  • Ancillary facilities: All ancillary facilities with a direct contract with Priority Health, except Borgess ancillary sites

Narrow network: MyPriority St. John Providence Network

Available to: Individuals under 65 who are members of individual MyPriority HMO narrow network plans

Identification: Member ID cards indicate "St. John Providence Network."

Counties: Macomb, Oakland and Wayne

In-network providers:

  • Crittenton Hospital and Medical Center
  • Providence Park Hospital: Novi Campus
  • Providence Park Hospital: Southfield Campus
  • St. John Hospital and Medical Center
  • St. John Macomb: Oakland Hospital – Oakland Center, Madison Heights Campus
  • St. John Macomb: Oakland Hospital, Warren Campus
  • St. John River District Hospital
  • All in-network ancillary providers and pharmacies

Physician network: 
All St. John Providence-employed physicians and those affiliated physicians who are contracted with Priority Health through the St. John Providence groups listed below:

  • St. John and St. John North Shore
  • St. John River District
  • St. John Oakland
  • St. John Macomb
  • St. John Cornerstone
  • Providence Hospital

Narrow network: MyPriority St. Joseph Mercy Health System Network

Available to: Individuals under 65 who are members of individual MyPriority HMO narrow network plans 

Identification: Member ID cards indicate "St. Joseph Mercy Health System Network."

Counties: Livingston, Macomb, Oakland, Washtenaw and Wayne counties

Network providers:

  • St. Joseph Mercy Ann Arbor 
  • St. Joseph Mercy Chelsea 
  • St. Joseph Mercy Livingston 
  • St. Joseph Mercy Livonia 
  • St. Mary Mercy Oakland 
  • All in-network ancillary providers and pharmacies 

Physician network

All physicians who are affiliated with St. Joseph Mercy groups as listed below:

  • Huron Valley Physician Association 
  • IHA 
  • Livingston Physician Organization 

Select physicians who are affiliated with St. Joseph Mercy groups as listed below:

  • Oakland Physician Network Services with St. Joseph Mercy Oakland designated as the primary hospital 
  • St. Mary Mercy physician practices
