Announcing our Primary Care Incentive Program contest

It's the first ever Priority Health Primary Care Incentive Program Contest. Work your incentive program reports this summer and you could win a catered lunch for your practice.

Summer is a great time to work your reports before the busy school season begins with sport and school physicals and the beginning of flu season. And, you're providing the quality care your patients need to improve their health and their lives and at the same time transforming the way health care is delivered.

How to participate 

All practices that participate in our CPC+ and PIP programs are eligible. It's easy to participate.

Identify your patients who need care, especially in these measures:

  • Controlled blood pressure
  • Controlled HbA1c less than or equal to 9.0%
  • Monitoring for nephropathy

Document all your hard work by completing the supplemental data information and claims data processed by Aug. 31 in order to be counted toward the contest.  Supplemental data includes:

  • HL7 – You're all set if you submit your data by Aug. 31 for the above three measures
  • Patient Profile
  • Report #70

You could win a catered lunch for your practice

July's PIP report will mark the benchmark for your practice, and September's PIP report will mark the end of the contest.  This allows us to measure your progress in July and August.  The three practices with the highest percentage increase in each of the measures listed above will win. 

The winners will be announced in October.
