Don't lose your Medicaid coverage. Take action!

As a Medicaid member, you need to renew your eligibility for benefits by your renewal date to keep your Medicaid coverage. Don't lose your benefits. Renew your eligibility for Medicaid when notified by the Michigan Department of Health and Human Services (MDHHS). If you have already renewed your benefits or are signed up for auto renewal, no action is needed. Contact your MDHHS caseworker with any questions.

Remember to take your health risk assessment

As a Healthy Michigan member, you may qualify for special incentives each year - like a reduction to the amount owed in the MI Health Account - just for completing your Health Risk Assessment when you visit your doctor. A Health Risk Assessment (HRA) is a list of questions to answer about your current health. Answer these questions the best you can and bring this form with you to your annual checkup. Your answers to these questions help us and your doctor better understand what you may need to be your healthiest. You can find the HRA form online at michigan.gov/mdhhs by searching "health risk assessment."

Health coaching for Healthy Michigan Plan members

Are you ready to improve your health and your life? Our certified health coaches can give you the information, guidance and encouragement you need to help you set goals and reach them. As a Healthy Michigan member, health coaching is convenient and is offered at no cost to you over the phone or through email. Your coach will reach out to you to plan sessions to fit your schedule. Coaches are flexible and available during daytime and evening hours. You can start or stop at any time.

Smoking Quitline

Quitting smoking is an important step in becoming healthier. If you're ready to quit, you may qualify for no-cost smoking cessation coaching and nicotine replacement therapy through the Michigan Tobacco Quitline.
