Annual wellness visit

An AWV is your yearly appointment with your primary care provider (PCP) to create or update your personalized prevention plan with the help of a Health Risk Assessment questionnaire. This plan helps you prevent illness based on your current health. You can also identify any health goals for the upcoming year.

Your provider may review your health history, check your vitals, screen for depression or update your list of medications and allergies.

Your AWV enhances your preventive health maintenance and helps you build a strong relationship with your provider, allowing for open communication.

Comprehensive physical exam

The purpose of a CPE is to screen for illness, assess the risk of future medical issues, encourage a healthy lifestyle and update vaccinations. A CPE typically involves a head-to-toe exam by your provider.

Your provider may check your vitals, perform a physical exam, test your balance, hearing or reflexes and give any needed vaccinations.

Your CPE includes a physical exam, one of the best ways to identify and treat health issues before they get worse. Plus, this visit is a covered benefit because you chose Priority Health. It’s “free to talk,” meaning you won’t be charged for the office visit, no matter what you discuss with your provider.

In-home health assessment

An IHHA is an in-home heath evaluation conducted by a licensed health care provider from our partner, Signify Health, to help identify any factors in your health care that should receive further evaluation. After the visit, both you and your primary care provider (PCP) will receive a summary of the visit along with any recommendations for care.

An IHHA typically lasts about 45 minutes. The provider will review your medical history, current medications and check your vitals. In some cases, the provider may leave behind a kit to collect a specimen, such as A1c for diabetics or a FIT kit to check for colon cancer. These visits are non-invasive and fully clothed.

Think of an IHHA as a "bonus check-up.” This doesn’t replace an annual wellness visit (AWV) or comprehensive physical exam (CPE). This service is completely covered by your plan at no cost to you.

Discussing fall prevention with your provider.

Did you know? Each year, three million older people are treated in emergency departments for fall injuries1. Be sure to discuss with your provider at your appointment how you can prevent the risk of falling. Download the “Understanding your risk for falls” PDF for a checklist of what to discuss with your provider, as well as tips for keeping your home safe, such as:

  • Place frequently used items in easy-to-reach places
  • Fix uneven floor surfaces
  • Use night lights
  • Ensure there are clear paths around your furniture
  • Beware of any and all pets
