(GRAND RAPIDS, Mich. — May 20, 2025) Nearly half of adults in the U.S. have high blood pressure, and only about 1 in 4 adults with high blood pressure have it under control.
During Hypertension Awareness Month, Priority Health announces “Priority Health at Home,” which offers in-home monitoring and support to members with a recent hospitalization or emergency room visit due to uncontrolled hypertension. The goal of Priority Health at Home is to help prevent unnecessary and costly emergency room visits, admissions and readmissions, while increasing overall quality of life. The program is a collaboration with Corewell Health’s Visiting Nurse Association (VNA).

By partnering with Corewell Health, we hope to help our members manage their condition more effectively and prevent future health crises.”
Home services include:
- In-home visits
- Remote blood pressure monitoring including installation, hands-on training and technical support
- Care coordination with a member's primary care providers and specialists
- Prescription management and support
- Long-term care planning
