Here are some frequently asked questions about PriorityREACH.

General

PriorityREACH is a mobile application, powered by Wellframe, that digitally connects you to your Priority Health Care Management Team. Your PriorityREACH care staff includes care managers, community health workers, and other health professionals who are ready to help you manage your health. Care management, delivered over the phone or through the PriorityREACH app, is included in your benefits at no additional cost. Participation is optional and won't affect your benefits.

Wellframe is an independent company providing technology that enables person-to-person Communication via text and improves the care management experience.

Eligibility

Priority Health will invite you to download the app. You’ll receive an access code, which you’ll need in order to complete the app registration. The access code may be provided to you via mail, email, or following a phone call with a Priority Health staff member. Currently, PriorityREACH is only available to our members with DSNP and Medicaid (non-pediatric) coverage.

No, a Care Manager will be assigned after you access the app. Call Priority Health Customer Service at 800.942.0954. to request an access code. Customer Service hours are Mon. - Thurs. 7:30 a.m. to 7 p.m., Fri. 9:00 a.m. to 5 p.m., and Sat. 8:30 a.m. to noon.

Creating an account

The access code you receive is required to complete your registration. It will be a series of letters and/or numbers provided by Priority Health that you’ll use to register for the PriorityREACH app.

Make sure you’re typing it correctly. It should have all lowercase letters and no spaces. It may include numbers. If you need assistance, call the Wellframe team at 1-844-452-4085 or send an email to help@wellframe.com.

Your password for the PriorityREACH app is the same as your Priority Health account password. Make sure you are entering it correctly or reset it by clicking the Forgot Password button in the app and following the steps. Your password can also be reset at member.priorityhealth.com/login and click Forgot Your Password?

If you’d like to create a Member Portal account before downloading PriorityREACH, go to member.priorityhealth.com/login and follow the prompts.

Please note, if you download the PriorityREACH app before setting up a Member Portal account, you will be prompted to create an account during the Priority REACH registration process.

App

With the Priority Health app, you have direct access to your member portal to view your health insurance benefits, track your claims, and get information about your plan. The PriorityREACH app gives you access to your Priority Health Care Management Team to help you manage a condition, recover from a recent surgery, and/or improve your health.

If you’re having trouble downloading the app, check to see that you have enough memory available, are connected to Wi-Fi, and have a mobile operating system that’s compatible with the app.

The app is compatible with Apple devices running iOS 14 and newer, including iPhone®, iPad®, and iPod touch®. It’s also compatible with smartphones and tablets running on AndroidTM 8 and newer, which run on brands such as Pixel, LG, Moto X, Nexus, HTC, and Sony. PriorityREACH cannot be used on your computer.

PriorityREACH is included in your Priority Health plan benefits at no additional cost to you.

The app may collect usage data about how you use our it, such as how you use the application, what content you read and favorite, content of messages, integration data, and device information. Wellframe uses this analytics information to improve the app for you, Priority Health, and other users.
