Managing your health just got easier. The new My Care Management tool lets you track and complete important tasks required by your Priority Health D-SNP, online and at home. Complete your health risk assessment (HRA) without ever leaving home—or wherever there's an internet connection.
How the My Care Management tool works
Your required tasks can now be done online. Here's how.
Complete your health risk assessment form
As part of your D-SNP, you're required to complete an HRA within 90 days of enrollment and once a year after that.
- Log in at member.priorityhealth.com.
- Scroll down the home page and select My Care Management, then click Continue.
- Click the My Health Assessments tile.
- Under Current Assessments, you'll find your health risk assessment form.
- Once you complete your assessment, it will appear under your Completed Assessments tab.
Sign off on your care plan
You or your caregiver can sign off on your care plan.
- Log in at member.priorityhealth.com.
- Scroll down the home page and select strong>My Care Management, then click Continue.
- Click the My Care Team tab.
