Managing your health just got easier. The new My Care Management tool lets you track and complete important tasks required by your Priority Health D-SNP, online and at home. Complete your health risk assessment (HRA) without ever leaving home—or wherever there's an internet connection.

How the My Care Management tool works

Your required tasks can now be done online. Here's how.

Complete your health risk assessment form

As part of your D-SNP, you're required to complete an HRA within 90 days of enrollment and once a year after that.

  1. Log in at member.priorityhealth.com.
  2. Scroll down the home page and select My Care Management, then click Continue.
  3. Click the My Health Assessments tile.
  4. Under Current Assessments, you'll find your health risk assessment form.
  5. Once you complete your assessment, it will appear under your Completed Assessments tab.

Sign off on your care plan

You or your caregiver can sign off on your care plan.

  1. Log in at member.priorityhealth.com.
  2. Scroll down the home page and select strong>My Care Management, then click Continue.
  3. Click the My Care Team tab.

What’s a Priority Health member account?

A Priority Health member account will give you access to all your personalized health information anywhere, anytime—all in one place.

I don’t have a Priority Health member account in order to access the My Care Management tool. How do I sign up?

You can log in to the Priority Health member account online or via the Priority Health app found on Google Play or the App Store. Sign up for a Priority Health member account at member.priorityhealth.com or by downloading the Priority Health app from the App Store or Google Play.

How do I access My Care Management within the Priority Health member account?

Once you're logged in to your Priority Health member account, scroll down on the home page and select My Care Management and click Continue. You are now in My Care Management.

If you your session is interrupted, please return to your Priority Health member account home screen and follow the instructions above.

What does the “My Messages” tile mean?
You'll be able to send and receive messages from your assigned care manager directly through the My Messages tile in your portal.

How long does it take to send/receive a message?

Give your care manager around 24-48 hours to reply to your message.

How do I know when I receive a message?

You'll need to log in to your member account and click into My Care Management to check and see if you have new messages waiting for you to read.

What does the “My Health Assessments” tile mean?

This tile is where you'll find the Health Risk Assessment that you're required to complete within the first 90 days of your membership, and at least once annually after that (as long as you are a Priority Health D-SNP member).

How do I complete the Health Risk Assessment (HRA) online and how long do I have to complete the HRA?

Your care manager will call you to inform you it's time to complete your HRA. As part of your D-SNP health plan, you're required to complete an HRA within 90 days of enrollment and once a year after that.

Log in at member.priorityhealth.com.

Scroll down the home page and select My Care Management, then click Continue.

Click the My Health Assessments tile.

Under Current Assessments, you'll find your health risk assessment form.

Once you complete your assessment, it will appear under your Completed Assessments tab.

Are there other ways to complete my survey besides online?

You also have the option to complete the survey:

  • By phone: Call 866.305.7110 (TTY 800.434.8300) then option 3
  • By mail:
    • New member: Complete the HRA form in your enrollment packet
    • Existing member: Complete the HRA form in your Annual Notice of Change documents
