1. Review your Priority Health benefit plan

2. Choose a Primary Care Physician
We'll walk you through the steps. (You can skip this step if you already have Priority Health and a PCP.)
Go to the PCP Selection instructions and form.

3. View the enrollment instructions
Go to the State of Michigan website.

4. Complete your enrollment
Use your MI HR Self-Service account (SSA). You must make your change(s) in your SSA no later than Friday, August 29, 2008.
