1. Review your Priority Health benefit plan

2. Choose a Primary Care Physician
We'll walk you through the steps. (You can skip this step if you already have Priority Health and a PCP.)
Go to the PCP Selection instructions and form.

3. View your benefit options
Go to the State of Michigan website and click on Health Maintenance Organization Plans.

4. Complete your enrollment
Use your MI HR Self-Service account (SSA). You must make your change(s) in your SSA no later than Friday, August 28, 2009.
