Enrollment steps

1. Review your Priority Health benefit plan
   • 2011 summary of benefits and copays for active employees (26KB PDF)
   • 2011 summary of benefits and copays for active employees hired on or after 04/01/2010 (150KB PDF)
   • 2011 summary of benefits and copays for retirees (61KB PDF)
2. Choose a primary care physician (PCP)
   We'll walk you through the steps. (You can skip this step if you already have Priority Health and a PCP.)
   Go to the PCP selection instructions and form.
3. View your benefit options
   Go to the State of Michigan website and click on Health Maintenance Organization Plans.
4. Complete your enrollment
   Use your MI HR Self-Service account (SSA). You must make your change(s) in your SSA no later than Tuesday, August 31, 2010.
