2021 PCP Incentive Program (PIP) webpage updates

Summary of what changed
Two new measure pages were added under the PIP program.
The MTM incentive measure page had significant changes to its measure components and program deadlines.

- Added the 2021 PCP Incentive Program manual and quick reference guide
- Posted quick links for 2021 incentive measures (MTM, SDoH, BHCC, and Care Management)
- Included past quality award detail and previous program year winners
Care opportunities using Patient Profile
- Added a quick link to Patient Profile
- Added a data upload schedule
- Updated requirements and instructions for special exception submission
Excluding non-adherent members
- Updated requirements and instructions for non-adherent submission
- Updated measure components and program deadlines
- Added new language to the attestation component
- Added new language to the collaborative practice agreement language
Social Determinates of Health (SDoH)
- Included measure components and program deadlines
- Added language for PCMH recognition in the program year
- Added details about screening for SDoH

Behavioral Health Collaborative Care (BHCC)
