Priority Health honored with 2019 MAHP Pinnacle award

(Lansing, Mich. – Sept. 28, 2019) Priority Health’s Medicaid Care Management team was awarded with the Michigan Association of Health Plans (MAHP) 2019 Pinnacle Award in the Community Outreach category for their Medically Complex Children Information Sharing Events project. Since 2001, MAHP has recognized health plans for achievements in addressing the challenges of a shifting health care environment through improvements in operations, clinical services, disease management and community outreach.

“Our team is thrilled to be recognized by MAHP for our hard work in the community outreach space,” said Sheila Wilson, director of care management for Medicaid and Individual Products at Priority Health. “We come to work every day with a passion for improving the lives of our members, and this honor means so much to us.” 

In working closely with their medically complex children (CSHCS – Children’s Special Health Care Services) and their families, Priority Health quickly identified the needs and gaps in knowledge related to care coordination between these families and the community providers that serve them. In response, Priority Health launched the Medically Complex Children’s Information Sharing Events. These quarterly events were created to provide a venue for health care providers and community partners that specifically work with the CSHCS population, to network, share information, establish referral pathways and work collaboratively. 

“The outcome was greater care coordination and the ability to access the many resources that are available and often underutilized within our community,” said Wilson. “Our goal remains to improve the welfare of our medically complex children and reduce the burden on their families.”
