Priority Health's purchase of Care Choices HMO and Preferred Choices PPO
received approval from the Office of Financial and Insurance Services (OFIS)
and became final on April 1, 2007. As a result, Priority Health now serves
an additional 120,000 members in Eastern Michigan, and welcomes 40 hospitals
and 7,300 physicians to its network.

If you need more information on a patient, call these numbers:
Collect Copayments On the front of the Care Choices and Preferred Choices ID cards you'll see what copayment (if any) is required. Collect the indicated copayment at the time of service. Submit Claims Bill Care Choices by submitting claims electronically or on paper. You will be paid as you would for any Priority Health member.
