Now Serving Care Choices Patients

Priority Health's purchase of Care Choices HMO and Preferred Choices PPO received approval from the Office of Financial and Insurance Services (OFIS) and became final on April 1, 2007. As a result, Priority Health now serves an additional 120,000 members in Eastern Michigan, and welcomes 40 hospitals and 7,300 physicians to its network.

What To Do When a Care Choices Patient Arrives
The relationship between our combined networks is as follows.
  • Priority Health HMO members now have access to see Care Choices HMO providers and the discount will apply.
  • Care Choices HMO members can see Priority Health HMO providers and the discount will apply.
  • Priority Health POS members now have access to see Care Choices HMO providers and the discount will apply.
  • Priority Health PPO members can see Preferred Choices PPO providers and the discount will apply.
  • Preferred Choices members can only see Preferred Choices providers due to the leasing arrangements contained in our contracts.
  • Priority Health HMO members cannot see Preferred Choices PPO providers and receive a discount.
  • Priority Health PPO members cannot see Care Choices HMO providers and receive a discount.
  • Care Choices HMO members cannot see Priority Health PPO providers and receive a discount.

Collect Copayments

On the front of the Care Choices and Preferred Choices ID cards you'll see what copayment (if any) is required. Collect the indicated copayment at the time of service.

Submit Claims
Bill Care Choices by submitting claims electronically or on paper. You will be paid as you would for any Priority Health member.
